Changing the course of events becomes possible when one person uses the power of their voice to describe a vision and a path to the vision so compelling to the hearts of the listeners that the head cannot find a useful argument against it.

Starting small with quick wins and demonstrating how small shifts lead to bigger changes will bring people along and build confidence
Sustained determination
Chipping away at it. Remembering that failure only happens when you have given up. If you hold onto the bigger picture then of course there will be stops and starts, recalculations and reviews and listening and learning along the way.
